Visit Delmarva Foundation Booth #832 for
Population Health Management Resources and Solutions

EASTON– November 13, 2014 – Delmarva Foundation, a nationally recognized leader in quality improvement, announced today it will exhibit at American Public Health Association (APHA) Annual Meeting & Exposition from November 15 – November 19 at the Ernest N. Morial New Orleans Convention Center, New Orleans, the preeminent annual public health gathering. Delmarva Foundation will showcase services and strategies available from the Population Health Center.

The APHA Annual Meeting & Exposition is the oldest and largest gathering of public health professionals in the world, attracting more than 13,000 national and international physicians, administrators, nurses, educators, researchers, epidemiologists and related health specialists. APHA's meeting addresses current and emerging health science, policy and practice issues in an effort to prevent disease and promote health.

During the event, booth visitors will view demonstrations of the Resource Access Card (RAC)—Delmarva Foundation’s innovative tool for community-based organizations to collect data and the Best Care Connector—an innovative care coordination service.

Delmarva Foundation will also host an interactive session, Connecting the Dots: Making Population Health Work - New Methods, New Ideas, in Hall B of the Morial Convention Center on November 19th at 9:00am. Attendees can gain an exclusive look at new population health innovations and services. Attendees will have the chance to win a brand new Kindle Fire HD.
